I do think it is great that you take such good care of your skin. I am one of those people that follows a strict routine and still manages to get breakouts. My only advice is not to apply moisturizer to the areas that break out overnight.
I was told by a dermatologist that when you sleep your body sweats out toxins (not to be gross but do you ever sometimes feel a little grimy in the morning before your shower even though you were just sleeping not doing exercise? this is the reason why), so adding moisturizer can block up your pores a bit and add to the blacheads.
Maybe use some withhazel at night on the breakout areas too.
Good luck.
